Housing Stabilization Services

MEDICAL ASSISTANCE (MA)

Housing Stabilization Services (HSS) can help you in three ways:

  1. A housing transition services provider helps you plan for, find, and move into housing. 

  2. A housing sustaining services provider helps you keep housing after move-in.

  3. A housing consultant helps develop a person-centered plan if you don't have MA case management.

Who Can Get Services

  • You have Medical Assistance

  • Are 18 or older

  • Have a disability or disabling condition:

    • SSI / SSDI​

    • Developmental disability

    • Substance Use Disorder (SUD)

    • Injury or illness with extended incapacitation

    • Mental illness

    • Learning disability

  • Are not getting similar services from other programs​

  • Are in one of the following situations:​

    • Homeless or at risk of homelessness​

    • Moving out of an institution, nursing facility, or certain other group settings

    • Ar risk of institutionalization

  • Are assessed to need help with at least one of these areas:​

    • Communication​

    • Mobility

    • Decision-making

    • Managing moods or behaviors

How to Get Services

Supportive Doctor

STEP 1: Get an Assessment from a...

  • Qualified professional, like a mental health professional, nurse practitioner, or doctor

  • Certified assessor (with a MnCHOICES assessment)

  • Senior care coordinator

  • Coordinated entry assessor

STEP 2: Make a Person-Centered Plan...

  • If you have a MA-Waiver case manager, senior care coordinator, or targeted case manager, that person will help you update your plan to include Housing Stabilization Services

  • If you don't have any of these people helping you, you can get help making a Housing Focused Person-Centered Plan from a registered housing consultant​

​No matter who helps you with your person-centered plan, you get to decide which provider will help you with housing transition and housing sustaining services

STEP 3: Get Approved...

  • Then, your assessment, proof of disability, and person-centered plan are sent to the Minnesota Department of Human Services (DHS) for approval

Reviewing CV

HSS Staff and families will develop strategies that foster long-term housing stability such as: ​

  • Tenant and property management relationships​

  • Establishing partnerships with tenants and landlords
